ECC Sunscreen Permission Form

I give permission to CBE's Early Childhood Center to (re)apply sunscreen to my child as needed.
Notice: This permission is valid for 12 months from the date of submission.

Please label your sunscreen with your child's first and last name to ensure no mix ups in the classroom.
